Appointments
Dr. Castillo is accepting new patients at the moment. To book an appointment, please call our office at (818) 880-4220. Once you’ve scheduled an appointment with our office, that time slot will be held exclusively for you. If you are unable to meet your booked appointment time for whatever reason, please call us 24 hours in advance to reschedule.
What to Bring to Your Initial Consultation
To ensure speedy dental treatment, please bring the following items with you on your initial visit to our practice:
- Insurance Information
- Photo Identification
- Any records relevant to your visit’s medical care
Please arrive 15 minutes before to the start of your booked session to complete any required documentation.
Insurance
We accept the majority of dental insurance policies. Please get familiar with your insurance plan, paying particular attention to co-pays. Our accounting department will complete all required documents for your dental benefits. We would like to emphasize, however, that your specific policy is a contract between you and your insurance carrier.
Please keep in mind that you are accountable for the full amount of your obligation if your insurance benefits result in less coverage than expected. We do not provide our services on the assumption that insurance companies will cover our entire charge. Our treatment is determined by what is best for you.
Payment
Unless otherwise agreed, payment is expected at the time services are given. This includes any co-payments or deductibles that may be applicable (amount not covered by your insurance).
Our office accepts cash, check, Visa, Discover, and MasterCard for your convenience.
